As a Home Health Aide, you will work closely with clients, families, and healthcare professionals to deliver high-quality care that enhances well-being and quality of life.
Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and observe patients’ health status, recording vital signs and changes in condition accurately in electronic health record (EHR) or medical documentation systems
- Provide companionship and emotional support
- Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and maintaining a safe environment
- Use medical terminology appropriately when documenting patient care and communicating with healthcare teams
- Operate assistive devices like Hoyer lifts safely to transfer patients and prevent injury
Qualifications
- Proven experience in home health care, assisted living, nursing homes, or long-term care environments
- Familiarity with EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems for accurate documentation
- Knowledge of dementia care, Alzheimer's care, hospice support, and working with individuals with developmental disabilities or disabilities
- Valid CPR certification
- Ability to perform patient observation and accurately record vital signs such as blood pressure, pulse, temperature, and respiration
- Experience with medical procedures including catheter care, medication administration, and hospice care support preferred
- Strong communication skills to collaborate effectively with healthcare teams while maintaining HIPAA compliance
