You'll provide comprehensive primary care throughout assisted living, memory care, skilled nursing, and senior housing communities across the Willmar region.
Every day presents a different mix of patients, clinical challenges, and opportunities to improve lives. You'll evaluate acute concerns, manage chronic diseases, coordinate care following hospitalization, and work closely with interdisciplinary teams to deliver proactive, patient-centered care.
Your work helps seniors stay healthier, avoid unnecessary hospitalizations, and continue living safely in the communities they call home.
What You'll Be Doing
- Conduct comprehensive patient assessments and routine provider visits
- Diagnose and manage acute and chronic medical conditions
- Develop individualized treatment plans
- Identify changes in condition and intervene proactively
- Coordinate care following hospitalization and rehabilitation
- Collaborate with physicians, nurses, therapists, caregivers, and facility leadership
- Educate patients and families on treatment plans and disease management
- Participate in specialty programs and quality improvement initiatives
- Help improve patient outcomes through preventative, evidence-based care

What We're Looking For
Required Qualifications
- Graduate of an accredited Physician Assistant program
- Active Minnesota PA-C license in good standing
- Strong assessment, diagnostic, and clinical decision-making skills
- Ability to practice independently while collaborating with physicians and interdisciplinary teams
- Passion for serving older adults and medically complex populations
- Reliable transportation and willingness to travel throughout Western Minnesota
