Overview
We are seeking a dedicated and knowledgeable Staff Pharmacist to join our healthcare team. The ideal candidate will possess a strong background in clinical pharmacy, with experience in acute care settings, long-term care (LTC) pharmacy, and a commitment to providing exceptional patient service. As a Staff Pharmacist, you will play a crucial role in patient assessment, medication management, and ensuring the safe and effective use of medications.
Responsibilities
- Provide comprehensive pharmaceutical care to patients, including medication therapy management and patient counseling.
- Conduct patient assessments to evaluate medication needs and outcomes.
- Collaborate with healthcare professionals to develop and implement individualized treatment plans.
- Compounding medications as required, following aseptic techniques and safety protocols.
- Administer immunizations and manage medication administration processes.
- Maintain accurate inventory management practices to ensure the availability of medications.
- Utilize pharmacy software and electronic health record (EHR) systems for documentation and patient care activities.
- Monitor patient responses to medications, particularly in critical care and pediatrics settings.
- Ensure compliance with HIPAA regulations and maintain confidentiality of patient information.
- Participate in pain management strategies and provide education on medication use for acute care patients.
Requirements
- Doctor of Pharmacy (Pharm.D.) degree from an accredited institution is required.
- Completion of a pharmacy residency or equivalent clinical pharmacy experience is preferred.
- Experience in LTC pharmacy or hospital settings is highly desirable.
- Strong knowledge of medical terminology, physiology, anatomy, and pharmacology.
- Proficiency in IV insertion and IV infusion techniques is an asset.
- Familiarity with EMR systems for efficient patient documentation is essential.
- Excellent communication skills with a focus on patient service and care coordination.
- Ability to work collaboratively within a multidisciplinary team environment.
