Duties
- Conduct comprehensive assessments to develop personalized care plans tailored to each patient’s needs.
- Deliver evidence-based physiotherapy interventions such as manual therapy, trigger point therapy, and mobility exercises.
- Manage discharge planning and coordinate with multidisciplinary teams to ensure seamless patient transitions across care settings.
- Document clinical findings accurately in electronic health records (EHR) systems following clinical documentation standards.
- Provide outpatient rehabilitation services in clinics or community settings, ensuring optimal patient outcomes.
- Educate patients and caregivers on injury prevention, activity modifications, and home exercise programs.
- Support inpatient and hospital-based care by assisting with acute care protocols and working within medical terminology frameworks.
Experience
- Proven experience practicing in a physical therapy role within outpatient clinics, hospitals, or home health environments.
- Strong knowledge of physiology, anatomy, and medical terminology relevant to diverse patient populations.
- Demonstrated ability to develop effective care plans for patients with orthopedics, neurology, geriatrics, pediatrics, or post-surgical needs.
- Familiarity with clinical documentation standards and EMR (Electronic Medical Records) or EMR systems management.
- Experience working with geriatric patients and providing home & community care services is highly desirable.
- Prior outpatient rehabilitation center experience or inpatient hospital experience preferred.
- Knowledge of discharge planning processes and working with multidisciplinary teams to optimize patient recovery.

- Inpatient Rehab Hospital: 1 year (Preferred)
License/Certification:
- Physical Therapy License in the state of Louisiana (Required)
