Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor patient health by taking vital signs such as blood pressure, pulse, and temperature; report changes to healthcare professionals
- Support medication administration following care plans and HIPAA regulations to ensure privacy and safety
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary
- Maintain a clean and safe environment by performing light cleaning duties and ensuring infection control protocols are followed
- Observe and document patient conditions, behaviors, and progress to inform care plans and coordinate with healthcare teams
- Use specialized equipment such as Hoyer lifts or wheelchairs to assist with mobility and prevent injury during transfers
Qualifications
- Prior experience in home health, assisted living, nursing homes, or group homes is highly preferred
- Knowledge of patient observation techniques, dementia care (including Alzheimer’s and memory care), autism support, hospice care, and working with individuals with developmental disabilities or disabilities in general
- Certifications such as CPR, First Aid, HIPAA compliance training are advantageous; willingness to obtain necessary certifications is expected
- Ability to lift heavy objects safely and perform physically demanding tasks as needed for resident support
- Strong communication skills for effective interaction with clients, families, and healthcare professionals
- Demonstrated reliability in following care plans accurately while maintaining confidentiality under HIPAA guidelines
- Experience working with diverse populations including seniors, individuals with disabilities or special needs is a plus
