Job Description
We are seeking an experienced Travel LPN/LVN - Home Health to provide nursing care to patients in their homes. The ideal candidate will have strong clinical judgment, excellent communication skills, and the ability to work independently while coordinating care with patients, families, and other healthcare professionals.
The LPN will provide, document, and evaluate nursing care while supporting health maintenance, illness prevention, rehabilitation, and recovery in a home health setting.
Responsibilities
- Provide skilled nursing care to patients in their homes.
- Assess patient conditions and provide appropriate nursing interventions.
- Monitor and evaluate patient and family responses to care.
- Coordinate care with physicians, nurses, healthcare providers, patients, and families.
- Maintain accurate and timely clinical documentation.
- Apply nursing principles and community health practices.
- Work independently with minimal supervision.
- Demonstrate strong clinical judgment, critical thinking, organization, and time-management skills.
- Maintain patient confidentiality and follow patient rights policies.
- Adapt to changing patient needs and caseload assignments.
- Follow facility policies, procedures, and customer service standards.
- Be flexible regarding work assignments and scheduling.
- Drive to patient homes safely, including during inclement weather when required.
Required Qualifications
- Active Massachusetts LPN license in hand at the time of submission.
- Minimum 1 year of acute nursing experience.
- Home Health experience required.
- Current BLS certification from the American Heart Association or American Red Cross.
- Valid driver's license and automobile insurance.
- Personal vehicle required.
- Ability to work independently in a home health environment.
- Strong communication, assessment, organization, and critical-thinking skills.
Preferred Qualifications
- Certified Home Health experience.
- OASIS proficiency.
- Infusion Therapy experience.
- Experience with self-scheduling of patient caseloads.
- Knowledge of home health reimbursement and regulatory requirements.
- Previous travel nursing experience.
Important Assignment Requirements
- Candidate must reside more than 50 miles from the facility.
- Preferred commute is 45 minutes or less from the Pittsfield office.
- Permanent address must be included with the candidate profile.
- All requested time off must be disclosed at the time of submission.
- Maximum of 5 days of RTO will be accepted.
- RTO requests submitted after submission or during the interview/offer process may not be approved.
- Traveler must have their own vehicle.
- Non-billable orientation: 8 hours.
- Weekend requirements: None; holiday coverage may be required.
- No regular on-call requirement is listed for this assignment.

Compensation
$2,350/week based on 40 hours
$20/hour taxable + $1,550 weekly non-taxable stipend*
*Stipend eligibility and amount are subject to applicable IRS requirements and candidate qualification.
