URGENT HIRING - RN Hospice (Care Manager)

Location: Barre, VT 05641

Pay Rate: $65/hr

Duration: 16 Weeks

Shift: Days (Monday–Friday, Some Weekend Coverage)

Hours: 36 Guaranteed Hours

REQUIREMENTS:

Active Vermont RN License
Minimum 1 Year of RN Experience
Home Health Experience Required
OASIS Experience Required
Case Management Experience Required
BLS/BCLS Certification
Homecare Homebase (HCHB) EMR Experience Preferred
Hospice Experience Preferred

Pay: $63.00 - $65.00 per hour

Work Location: In person
