At Dakota Home Care, we believe compassionate, high-quality care does not stop at the end of the business day.We are seeking a skilled and dependable On Call Nurse to support our clients, caregivers, and care teams during evenings, weekends, and after-hours situations. This role is ideal for a nurse who is calm under pressure, enjoys problem-solving, values relationship-based care, and is passionate about helping individuals safely remain at home.The On Call Nurse plays a critical role in supporting caregivers and families while ensuring clinical excellence and responsive care across our service area.
Schedule
On-call rotation may vary and will be coordinated with the Director of Nursing.
Typical on-call hours include:
- Weekdays: 4:30 PM – 8:00 AM
- Friday at 12:00 PM through Monday at 8:30 AM
What You’ll Do
- Respond to caregiver, client, and family calls in a timely and professional manner
- Triage and evaluate nursing concerns using clinical judgement and critical thinking
- Provide skilled nursing intervention and support in the home as needed
- Complete nursing visits for urgent or emergent situations
- Provide caregiver education, medication competencies, and clinical guidance
- Collaborate with Nurse Case Managers and healthcare providers
- Support emergent admissions during on-call hours when necessary
- Assist with after-hours scheduling support as needed
- Document all interactions accurately within agency systems
- Help ensure compliance with agency policies and state regulations
What We’re Looking For

- Is a licensed nurse (RN)
- Demonstrates strong clinical reasoning and nursing judgement
- Communicates calmly and effectively during urgent situations
- Is compassionate, dependable, and team-oriented
- Enjoys educating and supporting caregivers and families
- Can work independently while collaborating effectively with the care team
- Believes people are at the heart of healthcare

- Organized and responsive under pressure
- Home care experience is a plus
