This is a key leadership role responsible for overseeing psychosocial services, discharge planning, resident advocacy, and regulatory compliance. The ideal candidate understands the skilled nursing environment and can balance documentation, family communication, and survey readiness while maintaining strong resident-centered care.
Key Responsibilities
- Oversee all Social Services operations within the facility
- Complete psychosocial assessments and participate in care planning
- Manage discharge planning and transitions of care
- Handle resident grievances and support family communication
- Ensure compliance with state and federal regulations
- Maintain accurate documentation and survey readiness
- Collaborate with Nursing, Admissions, and interdisciplinary team
Qualifications
- Licensed Social Worker (LSW/LCSW) in New Jersey preferred
- Prior Director of Social Services experience in a Skilled Nursing Facility required
- Strong knowledge of discharge planning and Medicaid processes
- Experience with care plans and regulatory compliance
- Strong communication and organizational skills
