Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health by observing vital signs and reporting changes promptly
- Administer medications and manage medical documentation in compliance with HIPAA regulations
- Operate Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to accurately record care activities
- Provide specialized care such as Alzheimer's, dementia, memory care, hospice, or catheter care as needed
- Support clients with meal preparation, light cleaning, laundry, and home organization to promote a safe living environment
- Use medical equipment like Hoyer lifts and assist with transfers to ensure safety during mobility tasks
- Document patient progress and communicate effectively with healthcare teams to coordinate ongoing care plans
Skills
- Strong patient observation skills to detect changes in health status quickly and accurately
- Experience working in assisted living facilities, nursing homes, or home health environments
- Knowledge of medical terminology and medical documentation standards
- Proficiency with EMR/EHR systems for record keeping and reporting
- Ability to assist with complex procedures such as catheter care and medication administration
- Compassionate caregiver with experience working with individuals with developmental disabilities or dementia-related conditions
- CPR certification and familiarity with hospice care practices are highly desirable
- Skilled in patient care activities including vital signs measurement, mobility assistance, and personal hygiene support
- Capable of working independently while adhering to safety protocols and HIPAA privacy standards
- Experience with long-term care settings, home & community care, or live-in employment preferred
