Duties
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
- Monitor patients’ health by observing vital signs, noting changes in condition, and reporting findings accurately to healthcare professionals
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when required
- Administer medications following prescribed care plans while adhering to HIPAA regulations for patient privacy and confidentiality
- Maintain a clean and safe environment by performing light housekeeping tasks including cleaning, laundry, and organizing living spaces
- Support clients with cognitive challenges such as dementia or Alzheimer’s by implementing memory care techniques and engaging them in meaningful activities
- Document care provided, update care plans regularly, and communicate effectively with family members and healthcare teams to ensure consistent quality of care
Requirements
- Proven experience in home & community care, assisted living, nursing homes, or group homes is highly preferred
- Knowledge of patient observation techniques, medication administration, and vital signs monitoring
- Certifications such as CPR, First Aid, HIPAA compliance training, and any relevant nursing or caregiving credentials are required or highly desirable
- Ability to lift heavy objects safely using equipment like Hoyer lifts; physical stamina is essential for supporting mobility needs
- Experience working with individuals with developmental disabilities, autism spectrum disorder, dementia or memory care needs is advantageous
- Strong communication skills to collaborate with healthcare teams and family members effectively
