The Role:
We’re seeking a Nurse Practitioner – Primary Care to provide comprehensive, patient-centered medical care across the lifespan. In collaboration with physicians and interdisciplinary care teams, you’ll assess, diagnose, treat, and manage a wide range of acute and chronic conditions while promoting health maintenance and disease prevention. Your clinical expertise and patient-focused approach will support our mission of delivering high-quality primary care in a compassionate and efficient environment.
What You’ll Do:

Perform comprehensive physical examinations and health assessments.

Diagnose and manage acute and chronic illnesses.

Order, interpret, and evaluate diagnostic tests, including labs and imaging.

Develop and implement individualized treatment plans.

Prescribe medications and therapiesin accordance withstate regulations.

Provide preventive care, including immunizations and routine screenings.

Manage chronic conditions such as diabetes, hypertension, asthma, and heart disease.

Educate patients and families on disease management, lifestyle modifications, and preventive care.

Perform minor office procedures such as suturing, wound care, joint injections, and skin biopsies.

Coordinate care with specialists and other healthcare professionals.

Document patient encounters accurately andtimelyin the electronic health record (EHR).

Participate in quality improvement initiatives and population health management.

Other relevant duties as assigned by management.
What We’re Looking For:

Master’s or Doctorate degree in Nursing (MSN or DNP).

Graduate of an accredited Nurse Practitioner program.
-
Current Registered Nurse (RN) license.
-
Current Nurse Practitioner license in the practicing state.

Nationalboard certification(AANP or ANCC) – Family NP or Adult-Gerontology NP.

DEA registration (if prescribing controlled substances).
-
BLS/ACLS may be required depending onsetting.

Strong clinical assessment and diagnostic skills.

Excellent communication and patient education abilities.

Knowledge of evidence-based practice guidelines.

Ability to work independently and collaboratively.

Proficiencywith electronic medical records (EHR/EMR).

Cultural competency anda patient-centered care focus.
Work Environment:

Outpatient primary care clinic.
-
May include community health centers, private practices, or hospital-affiliated clinics.

Typical schedule: Monday–Friday, 8am–5pm.
