The Women’s Health Triage Nurse plays a central role in clinical communication, care coordination, and operational support. This position requires strong clinical discernment, the ability to triage women’s health concerns appropriately, and a high level of organization in managing ongoing patient needs.
This role serves as a primary point of contact for patients seeking guidance, clarification, and ongoing support related to cycles, hormones, fertility, perimenopause, menopause, and general women’s wellness. This is a full‑time, in‑person position.
Triage experience — phone, portal, and email‑based clinical assessment
Functional medicine experience (personal or professional)
Ability to trend labs and identify patterns over time
Strong communication skills — clear, concise, patient‑centered
Medication knowledge — common adult medications, interactions, and safety considerations
Assess incoming patient concerns via phone, portal, and email using structured triage protocols
Determine appropriate level of care and escalate to providers when clinically indicated
Provide clear, informed guidance to patients within scope of practice
Support acute and non‑urgent clinical inquiries with sound judgment and professionalism
Facilitate follow‑up on labs, treatment plans, and provider recommendations
Coordinate referrals, imaging, and adjunctive therapies aligned with functional medicine care plans
Ensure clarity and continuity in patient care plans across all touchpoints
Assist in tracking patient progress and clinical outcomes
Document all patient interactions thoroughly and accurately within the EMR
Manage provider inbox workflows and support timely communication
Assist with pre‑visit preparation and post‑visit follow‑up processes
Contribute to the development and maintenance of clinical protocols and workflows
Active IL RN license required
Adult nursing and/or triage experience required
Demonstrated clinical judgment and ability to prioritize patient needs effectively
Proficiency with EMR systems and patient communication platforms
Strong written and verbal communication skills
Prior experience or strong interest in functional, integrative, or holistic medicine required
High level of organization and attention to detail
Ability to manage multiple clinical workflows concurrently
