Community Inclusion Coordinator - 35 Hrs/wk (CBDH)

Shift: Monday – Friday, 7:00A-3:00P (flexible)
Salary: $26.68/hr

Qualifications:
- Associate’s Degree.
- Clean and valid NYS Driver’s License required.
- AMAP Certification required (paid training provided).
Responsibilities include, but not limited to:
- Transport participants to/from program on a daily basis, using agency vehicles.
- Provide coverage, i.e. AM/PM transport routes.
- Fulfill AMAP responsibilities as needed.
- Conduct community site assessments to ensure safe accessibility.
- Coordinate, implement and evaluate person-centered community leisure and volunteer activities on a daily basis.
- Promote greater independence, inclusion, and ensure activities are consistent with participants' desires and valued outcomes.
- Complete electronic data (Therap).
- Responsible for programmatic paperwork including but not limited to staff action plans and ISP programs, chart reviews, monthly summary notes, daily activity notes, and daily trip logs.
- Attend case meetings and/or contribute feedback to participants' overall progress in day program.
- Accompany participants to medical appointments/emergencies and relay information to the appropriate team member.
- Communicate with parents/correspondents/advocates/care managers.
- Provide status reports to supervisors and peers.
- Provide training, feedback and assistance to staff as needed.
- Ensure ADL needs of participants are met including but not limited to their safety and well-being.
- Responsible for ordering/obtaining supplies for activities.
- Conduct and participate in water activities during the summer season.
- Incorporate Code of Ethics, Core Competencies and Empowerment Strategies in their daily professional responsibilities.
- Ensure compliance with OPWDD regulations and Agency Policy & Procedures, to include but not limited to training on behavioral plans/strategies & protocols.

The Cerebral Palsy Association of Nassau County is a nonprofit, multi-faceted health agency serving over 1,500 children and adults with cerebral palsy, developmental and other disabilities.
Day Habilitation is a year-round program serving individuals ages 21 and older who are developmentally disabled. Day Hab is a community outreach program where trained and qualified individuals with developmental disabilities provide local non- profit businesses with community service.
