MDS Nurse – Skilled Nursing Facility (SNF)

Desert Willow Post Acute is seeking a dedicated and detail-oriented MDS Nurse (Minimum Data Set Nurse) to join our Skilled Nursing Facility team. The MDS Nurse is responsible for accurate and timely completion of MDS assessments, care plan development, and regulatory compliance, ensuring residents receive high-quality, individualized care. This role works closely with nursing staff, interdisciplinary teams, and leadership to optimize clinical outcomes and facility performance.
Requirements & Qualifications
- Active RN or LVN license in the state of employment
- MDS Certification
- Current CPR/BLS certification - CDI, RAC-CT, or AHCA MDS
- Prior experience with MDS assessments and care plan development in a Skilled Nursing or Post-Acute Care setting
- Knowledge of federal and state long-term care regulations
- Proficiency with electronic medical records (EMR) and MDS software
- Strong attention to detail, analytical skills, and ability to meet deadlines
- Excellent communication and teamwork skills
Key Responsibilities
- Complete accurate and timely MDS assessments for all residents per regulatory requirements
- Develop individualized care plans in collaboration with the interdisciplinary team
- Review clinical records to ensure documentation supports MDS coding and quality measures
- Ensure compliance with federal, state, and facility regulations
- Collaborate with nursing, therapy, dietary, and social services to optimize resident outcomes
- Provide education and guidance to nursing staff regarding MDS processes and regulatory compliance
- Participate in quality improvement initiatives and reporting for regulatory surveys and audits
- Monitor and analyze MDS data for accuracy and trends impacting resident care and reimbursement
