We are seeking a licensed or license-eligible Social Worker/Therapist to join our Adolescent Unit as a Patient Care Manager (PCM) at our East facility.
This is a full-time position working primarily with adolescents in an inpatient psychiatric setting. The ideal candidate thrives in a fast-paced environment, is passionate about working with youth, and values teamwork and quality patient care.

8:00 AM – 4:00 PM

Some flexibility with hours based on department needs

Manage and coordinate care for an assigned adolescent patient caseload

Provide group therapy and psychoeducation
-
Complete accurate and timely clinical documentation
-
Collaborate with interdisciplinary team members to support patient outcomes

Participate in utilization review processes
-
Deliver excellent customer service to patients and families
Required:
-
Master's degree in Social Work, Counseling, or related field
-
Must be at least 21 years of age
-
Ability to pass background check, drug screening, and obtain fingerprint clearance
Preferred:
-
Licensed or license-eligible in Arizona (LAC, LPC, LMSW, LCSW, LMFT, LISAC)

Candidates working toward licensure (MSW, MS, LMSW, LAC, LASAC) will be considered

Minimum of 2 years of experience in a behavioral health setting
