As a Full-Time MDS Coordinator you play a critical role in ensuring accurate clinical documentation, regulatory compliance, and appropriate reimbursement—while supporting high-quality, resident-centered care.

Coordinate and complete MDS assessments from start to finish, making sure everything is accurate and submitted on time
Manage daily and weekly MDS schedules (ARDs), keeping track of due dates and making sure nothing falls behind
Review resident charts, therapy notes, and nursing documentation to ensure assessments reflect the full clinical picture
Work closely with nursing, therapy, dietary, and social services to gather the right information and keep documentation aligned
Lead or participate in care plan meetings, helping ensure each resident’s plan reflects their needs and goals
Identify missing or inconsistent documentation and follow up with the team to resolve it before submission
Monitor Case Mix (CMI) and support accurate reimbursement by ensuring documentation supports the services being provided
Assist with audits, surveys, and quality reviews by maintaining organized, compliant, and survey-ready documentation
Help educate and support staff on documentation best practices to improve accuracy and outcomes
Serve as a key point of contact for anything related to MDS, care planning, and regulatory compliance
Current, active Registered Nurse (RN) state license OR LPN actively enrolled in an RN program
Long-term care experience preferred
Prior MDS Coordinator experience preferred, but training is available for the right candidate
Strong written and verbal communication skills
Detail-oriented, organized, and deadline-driven
Positive, dependable, and team-focused attitude
