Duties
- Assist clients with activities of daily living (ADLs), including personal hygiene, grooming, dressing, and mobility support using manual handling techniques and patient transfer methods such as Hoyer lifts.
- Prepare nutritious meals and assist with meal planning to meet individual dietary needs.
- Support clients during community outings or appointments by providing personal transportation assistance and ensuring safety throughout activities.
- Maintain detailed documentation of care provided, including patient observations, medication administration records, and incident reports.
- Engage in patient monitoring for individuals with dementia, Alzheimer's disease, or other neurodevelopmental disorders, ensuring their safety and comfort at all times.
Experience
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred.
- Demonstrated ability to provide patient care with patience and compassion in settings involving dementia care, memory care, hospice care, or geriatrics.
- Experience working with individuals with autism or intellectual disabilities supports your ability to adapt care strategies effectively.
- Knowledge of medication administration procedures and first aid/CPR certification is essential for ensuring client safety.
- Familiarity with care plans development and implementation helps deliver personalized support aligned with each client’s needs.
- Hands-on experience with patient transfer methods such as Hoyer lifts and patient personal care assistance enhances your ability to perform manual handling safely.
- Ability to communicate clearly with clients, families, and healthcare teams while maintaining confidentiality under HIPAA guidelines.
