Your Role: Making a Meaningful Difference
The Client Intake Coordinator plays a vital role in connecting families to Cariño Family Health & Wellness Programs, including Parents as Teachers (PAT) and Nurse-Family Partnership (NFP).
This position ensures a welcoming, smooth, and responsive intake experience by managing referrals, conducting family outreach, verifying eligibility, and coordinating initial visits. Serving as a key communication liaison between families, providers, and program staff, the Intake Coordinator helps remove barriers to enrollment and supports home visitors in delivering services efficiently.
This role is ideal for someone who is organized, compassionate, and passionate about helping families access meaningful support.
Key ResponsibilitiesReferral Management & Family Outreach
- Receive and track incoming referrals.
- Conduct timely, family-friendly outreach to explain services and begin the intake process.
Eligibility & Documentation
- Gather and verify eligibility information (age, location, insurance, etc.).
- Accurately enter and maintain data in tracking systems and databases.
Scheduling Support
- Coordinate and schedule initial visits with Parent Educators or Nurses.
- Manage calendars and prepare intake documentation.
Provider & Partner Communication
- Serve as a communication bridge between Cariño and referral partners.
- Provide updates, support warm handoffs, and share referral status.
Home Visitor Support
- Assist home visitors with scheduling and sharing relevant family information.
- Help identify and address barriers to enrollment.
Cross-Team Collaboration
- Participate in team meetings, referral reviews, and quality improvement efforts.
General Office Support
- Respond to family inquiries.
- Support re-engagement efforts and assist with special projects as needed.
What We Are Looking For (Requirements)
Education: High School Diploma or higher.
Experience: Experience working in a busy office or administrative setting.
Technical Skills:
- Knowledge of office management systems and procedures.
- Proficiency in Microsoft Office (Word, Excel, Outlook, Teams).
Professional Skills:
- Strong organizational, communication, and interpersonal skills.
- Detail-oriented, dependable team player.
- Ability to clearly organize and communicate information to diverse audiences.
Language Skills: Bilingual English/Spanish preferred.
Work Environment & Physical Requirements
This position may require:
- Lifting and carrying up to 40 pounds.
- Standing and walking for extended periods.
- Bending, stooping, crouching, kneeling, and reaching.
- Performing physically active tasks during the workday.
- Navigating stairs, ramps, or uneven surfaces if needed.
- If driving is required, a valid driver’s license and safe driving ability.
