Job Summary
Pay would be 70 a visit. With mileage reimbursement. We are seeking a dedicated and energetic Mobile Physical Therapist to join our dynamic healthcare team. In this role, you will provide comprehensive physical therapy services directly in patients’ homes and community settings, promoting recovery, mobility, and independence. Your expertise will help develop personalized care plans, deliver targeted treatments, and collaborate with a multidisciplinary team to ensure optimal patient outcomes. Responsibilities
- Conduct thorough assessments of patients’ physical conditions in home and community environments to develop tailored treatment plans.
- Deliver hands-on therapy interventions such as therapeutic exercises, trigger point therapy, and mobility training to improve function and reduce pain.
- Coordinate discharge planning and collaborate with healthcare professionals to ensure seamless patient transitions across care settings.
- Maintain accurate clinical documentation adhering to standards for medical terminology, physiotherapy protocols, and electronic health records (EHR) management.
- Educate patients and caregivers on treatment techniques, safety precautions, and home exercise programs to support ongoing rehabilitation.
- Monitor patient progress regularly, adjusting treatment plans as needed based on clinical evaluations and physiology knowledge.
- Assist with outpatient clinic activities or inpatient hospital stays when required, ensuring consistent quality of care across settings.
Qualifications
- Valid licensure as a Physical Therapist with practicing in a physical therapy role in relevant jurisdictions.
- Proven experience in outpatient clinics, inpatient hospital settings, or home health environments with a focus on patient care.
- Strong knowledge of anatomy, physiology, orthopedics, neurology, geriatrics, pediatrics, and related medical terminology.
- Experience with discharge planning, acute care procedures, and outpatient rehabilitation center protocols.
- Proficiency in clinical documentation standards and electronic health records (EHR/EMR systems).
- Ability to work independently while collaborating effectively with healthcare professionals such as physicians and nurses.
- Excellent client communication skills to educate patients and families about treatment plans and rehabilitation goals.
