Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health by taking vital signs such as blood pressure, pulse, and temperature; report changes to healthcare professionals
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding if necessary
- Administer medications accurately following care plans and HIPAA regulations to ensure patient safety and confidentiality
- Support patients with mobility using equipment such as Hoyer lifts or transfer belts; perform heavy lifting when required
- Maintain a clean and safe environment through light housekeeping tasks like cleaning, laundry, and organizing living spaces
- Observe and document patient conditions, behaviors, and progress to inform care plans and communicate effectively with healthcare teams
Requirements
- Prior experience in home care, assisted living, nursing home settings, or group homes is highly preferred
- Certification in First Aid and CPR is required; additional training in Alzheimer's care, dementia care, hospice care, or working with individuals with developmental disabilities is a plus
- Knowledge of HIPAA regulations to ensure patient privacy and confidentiality at all times
- Ability to work independently while following detailed care plans; excellent observational skills for patient monitoring
- Physical stamina for activities such as heavy lifting, assisting with transfers, and long periods of standing or walking
- Strong communication skills to effectively interact with clients, families, and healthcare providers
- Valid driver’s license and reliable transportation are often necessary for home visits
