Join our dedicated team as a Home Health Aide, where you will play a vital role in providing compassionate, personalized care to individuals in their homes. This position offers a rewarding opportunity to make a meaningful difference in the lives of patients by assisting with daily activities, ensuring their safety, and promoting independence. Working Monday through Friday from 9 am to 5 pm, you will be an essential part of a supportive care environment that values professionalism, empathy, and excellence in patient care.
Duties
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor and observe patient health status, including vital signs such as blood pressure, pulse, and temperature
- Assist with Medication Reminders
- Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques and behavioral management
- Manage transfers safely to prevent injury
- Support patients during hospice or palliative care needs with compassion and respect
- Perform light housekeeping tasks including cleaning, laundry, meal preparation, and maintaining a safe environment in compliance with HIPAA regulations
- Document all patient interactions accurately in medical records and ensure confidentiality at all times
- Collaborate with healthcare teams to coordinate patient care plans and communicate any changes or concerns effectively
- Engage with patients in home & community settings to promote well-being and independence while respecting their dignity
Qualifications
- Proven experience working as a Home Health Aide or in long-term care settings such as nursing homes or assisted living facilities
- Strong knowledge of medical terminology, patient observation techniques, and caregiving best practices
- Familiarity with EMR/EHR systems for documentation purposes
- Certification in CPR preferred; additional certifications such as hospice care, dementia care, or medication administration are a plus
- Experience working with people with developmental disabilities or disabilities is highly valued
- Ability to perform physical tasks and assisting with mobility needs
- Excellent communication skills to interact effectively with patients, families, and healthcare professionals
- Knowledge of HIPAA regulations to ensure patient privacy and confidentiality at all times
- Ability to work independently while maintaining high standards of safety and professionalism
