Overview
The Community Outreach Coordinator will directly educate a prospective participant through the onboarding process leading to enrollment in the Program of All-inclusive Care for the Elderly (PACE). This person will work closely with referral sources, account managers, clinical intake and provider relations to ensure all tasks are efficiently completed associated with the enrollment process. The ideal candidate will possess strong leadership skills and a passion for community engagement, ensuring that our programs effectively meet the needs of the populations we serve.
Responsibilities
- Establish relationships in the community with potential referral sources
- Provide community education on the PACE model and benefits of Kinship PACE
- Correspond directly with the prospective participants to provide planned education and qualification screening.
- Respond to all incoming/outgoing calls, data entry, and emails. schedule assessments and mail information to prospective participants.
- Meet the departmental productivity goals specified by management for the growth of the company.
- Become an expert on Kinship PACE program eligibility requirements, services, and benefits.
- Address eligibility issues, application processing, and referrals to the state Medicaid system to resolve potential participant issues.
- Work closely with all departments onboard and retain participants.
- Obtain referral details to assess and determine appropriate next steps for expeditious enrollment processing.
- Evaluate factors unique to each referral, including, but not limited to, their clinical condition, financial situation, entitlements status, and home support systems.
- Use knowledge obtained through qualification screening to determine which government agencies and team members should be assigned to the prospect or caregiver in the next step of enrollment.
- Verify plan and entitlement status utilizing State Medicaid and CMS resources and functions.
- Assist potential participants with Medicaid applications as needed
- Meet with prospective participants and/or caregivers at the PACE Center or in their homes.
- Enter calls/inquiries into the central database for tracking and follow-up and schedule appointments with the prospect/caregiver.
- Monitor and report Intake team performance measures to improve delivery and execution of onboarding procedures.
- Maintain and generate reports to measure performance and quality of work as set forth by Kinship PACE.
EDUCATION & EXPERIENCE REQUIRED
- Associate degree or equivalent years of experience required.
- Minimum of 1-year experience in an equivalent position; healthcare field.
- 1 years’ experience working with the elderly population.
- Ability to maintain confidentiality and strong attention to detail while working in a fast-paced environment.
- Proficient in Microsoft Office Suite: Word, Excel, and PowerPoint
- Excellent communication skills, written and verbal.
- Excellent data analysis, organization, and relationship-building skills.
