What You'll Do
As an LPN Field Nurse, you'll provide skilled nursing care under the supervision of a Registered Nurse in accordance with physician orders and individualized plans of care.
Responsibilities include:
- Providing skilled nursing care in patients' homes
- Monitoring patient conditions and reporting changes to the RN and physician
- Educating patients and caregivers on medications, disease management, and safety
- Accurately documenting visits in the electronic medical record
- Communicating with physicians and the interdisciplinary care team
- Delivering compassionate, patient-centered care that promotes independence and quality of life
Qualifications
- Current Tennessee Licensed Practical Nurse (LPN) license
- Associate degree in nursing (minimum requirement)
- Current driver's license and dependable transportation with liability insurance
- CPR certification (or willingness to obtain upon hire)
- Ability to work independently and make sound clinical decisions
- Strong communication and interpersonal skills
- Commitment to maintaining patient privacy and HIPAA compliance
- Ability to work respectfully with patients from diverse backgrounds and varying levels of cognitive ability
