Job Summary
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting clients through various challenges, providing guidance, resources, and advocacy to ensure their needs are met. This position requires a strong understanding of clinical practices and the ability to work collaboratively with clients, families, and other professionals in a fast-paced environment.
Responsibilities
- CANS & ANSA Certification Required
- Conduct comprehensive assessments to determine client needs and develop individualized care plans.
- Provide clinical counseling and psychotherapy to clients, utilizing techniques such as Cognitive Behavioral Therapy and Crisis Intervention.
- Maintain confidentiality and adhere to HIPAA regulations while managing sensitive client information.
- Collaborate with healthcare providers, social services, and community resources to coordinate care effectively.
- Monitor client progress and adjust care plans as necessary based on ongoing evaluations.
- Engage in child family counseling sessions when appropriate, ensuring a holistic approach to client care.
- Conduct diagnostic evaluations to identify mental health issues and develop appropriate treatment strategies.
- Stay informed about current research and best practices in case management and mental health services.
- Support clients dealing with chronic pain through specialized counseling techniques.
Requirements
- A degree in social work, psychology, or a related field is preferred.
- Strong knowledge of medical terminology and clinical practices relevant to case management.
- Experience in crisis intervention and the ability to handle high-pressure situations effectively.
- Familiarity with various therapeutic modalities, including Psychotherapy and Cognitive Behavioral Therapy.
- Excellent communication skills, both verbal and written, with the ability to build rapport with clients from diverse backgrounds.
- Ability to work independently as well as part of a multidisciplinary team.
- Prior experience in child family counseling is a plus but not required.
- Commitment to ongoing professional development and staying current with industry trends.
