Home Helpers of Santa Clara Valley is currently seeking a motivated, assertive Client Care Coordinator to serve Santa Clara County and its surrounding territory.
The Care Coordinator will provide efficient, comprehensive, and effective care management services to older adult and disabled residents, which include a comprehensive assessment of the clients, development of a written care plan with measurable outcomes, arrangement and coordination of services, and client follow-up to monitor the effectiveness of the services arranged and assess the need for additional services.
Essential Duties and Responsibilities
- Conducting initial non-medical home safety assessments and developing Individualized Service Plans for new clients.
- Ensure all intake documentation is properly executed before the commencement of care.
- Conduct regular quality assurance (QA) check-in calls with clients and authorized family members to ensure satisfaction and adjust service plans as needs change.
- Coordinate with the staffing coordinator to ensure the perfect match between client needs/preferences and caregiver skills.
- Work alongside the staffing coordinator to monitor the Electronic Visit Verification (EVV) system for missed clock-ins, late arrivals, or shift exceptions, and proactively manage emergency coverage to prevent gaps in care.
- Ensure caregivers are properly briefed on the specific needs of their assigned clients, strictly enforcing the boundary of non-medical care (e.g., medication reminders only).
- Maintain accurate and up-to-date client records in compliance with California Department of Social Services (CDSS) and Joint Commission standards.
- Act as the first point of contact for client or family grievances, ensuring all complaints are logged and escalated according to our strict 14-day resolution timeline.
- Review caregiver daily visit notes to ensure care is going well and to see if there are any changes in the client's condition.
- Coordinate the deployment, client onboarding, and basic troubleshooting of Aloe Care Personal Emergency Response Systems (PERS).
- Serve as a professional liaison to our community partners, including coordinating schedules and communicating non-medical updates with the Veterans Affairs (Tri-west) program and staff at the Saratoga Retirement Facility.
Qualifications & Requirements
- Minimum of 6m–1 year of experience in home care coordination, scheduling, customer service, or a closely related healthcare administration role.
- Strong understanding of non-medical home care operations, HIPAA privacy rules, and California Home Care Aide registry requirements.
- Exceptional organizational, multi-tasking, and problem-solving skills.
- Excellent verbal and written communication skills to interact professionally with elderly clients, family members, and diverse field staff.
- Proficiency in home care scheduling software (e.g., ClearCare, WellSky) and standard office applications (Microsoft Office Suite / Google Workspace).
- Bilingual language skills are a must.
- Previous case management experience strongly preferred; knowledgeable of community resources and needs of client population.
- Must pass a comprehensive background check. Must have a valid California driver's license and reliable transportation for field visits.
- Must travel throughout the designated service territory.
