Function/Skills:
QIDP organizes and provides program content for social events for our organization and individuals. Being an Activity Specialist is familiar with standard concepts, practices, and procedures working with the developmentally disabled population. Relies on limited experience and judgment to plan and accomplish goals while performing a variety of tasks. Works under general supervision; reports to the Day Habilitation Coordinator.
Duties and Responsibilities:
- Report to the Day Program Coordinator. Day Program QIDP is responsible to assist the Day Program Supervisor in the day to day operation of the Day Program.
- Complete accurately the following routine written work: Staff Action Plans, documentation of services delivered contemporaneously, Individuals daily attendance, progress notes, any documentation required by supervisor and/or administration.
- Attend Comm Hab R meetings and update SAP plans per meeting.
- Implement treatment plans along with Program Supervisor
- Participate in cross program service reviews that support the planning and coordination of service delivery and service closure.
- Actively participated in all Individual conference meeting and treatment plans.
- Create monthly calendar for recreation/volunteer/holidays/birthdays/and indoor events Circulate via email to all programs. Review calendar for editing as needed.
- Request and maintain photos/ consent for phot usage. Create scrapbooks for day program.
- Register Day Program staff to ensure compliance with all initial and annual training requirements for Day Hab Staff per OPWDD and ILA standards.
- Manages the Day Program in the absence of the Supervisor and report to the Area Coordinator.
- Ensure safety and sanitation in the Day Habilitation Hub and that adequate support services (vendors, suppliers) are maintained. Per COVID-19 protocol. (see safety plan for cleaning guidelines)
- In conjunction with the Supervisor, completes and administers all performance evaluations of union staff.
- Participates in administering the Agency’s progressive disciplinary program.
- Conducts suspensions and participated in employment terminations.
- Participates in the grievance process with union employees.
- Ensure appropriate staff for assignments specific to daily task.
- Attends staff / agency meetings as required
- As part of the hiring process, attends job/career fairs and open house with the recruitment’s specialist.
- Provide emergency coverage of the program as the occasion may arise.
- Responsible for planning Individual daily activities, and liaison with the community.
- Develop opportunities for recruiting Individuals for the program.
- Responsible for the training, guiding and motivation of staff in the implementation of the Individuals’ treatment plans:
- Responsible for good community relations especially the immediate surrounding community.
- Perform additional duties as required.
- Completes other duties as directed by the Program Supervisor.
Requirements:
- B.A. or B.S. degree required
- 1-3 years previous experience preferred
- Experience in Day Habilitation preferred
- Strong Writing Skills

Pay: $50,000.00 - $60,000.00 per year
