Attending/Always Homecare- Brooklyn Location- Is seeking an experienced Spanish/English Intake Supervisor to join its growing team. The Intake Supervisor is responsible for overseeing the day-to-day operations of the Intake department and maintaining relationships with the business development representatives from the Managed Long-Term Care (MLTC) agencies, CHHA agencies and other referral sources.

Responsibilities and Duties:
- Assess patient referrals to assure they are appropriate for those services provided by Always Home Care Services, Inc.
- Scheduling quarterly meetings with the representatives from our contracts for thorough reviews of contractual relations.
- Communicate with all relevant parties (i.e. patient, referral source & agency staff) to expedite the processing of patient referrals.
- Assure the accuracy of patient/referral information (i.e. patient information, demographics, referral source & other agency mandates).
- Ensure a seamless, complete and effective enrollment process for all prospective clients into our many MLTC or CHHA programs.
- Confirming Medicaid eligibility in E-paces for prospective members.
- Assist with the tracking of all referrals to assure appropriate follow up from inception of referral to admission.
- Provide reports which encompasses, discharges, admissions, number or referrals on a weekly basis.
- Participate in weekly management meetings to discuss the Intake department as well as share ideas to ensure its efficiency.
- Ensures all referrals are being processed and followed up, while ascertaining that customer service and referral sources are satisfied
- Respond to patient and agency requests for additional services or transfer of services to another insurance plan.
- Provide supervision and oversight to the Intake coordinators as needed.
- Assist with the development of new referrals in conjunction with agency MLTCP plan business development initiatives.
- Assist with obtaining MLTCP/MCO authorizations for services.

- Homecare industry experience REQUIRED.
- Bilingual Spanish speaking is a must.
- HHA Exchange experience a must!!!
- Bolt experience a plus
