MDS Coordinator
Wayne County Rehab is seeking an MDS Coordinator for our Skilled Nursing Facility.
Duties Include:
Completing accurate assessments, MDS & care plans as assigned.
Initiating care plans and supporting activities as assigned.
Maintaining & updating all care plans and assessments as required.
Ability to identify problems and to communicate those problems to the DON and staff concerned
Always Protecting the confidentiality of Resident & Facility information.
Requirements Include:
Valid NY State RN License
Long Term Care Experience Required!
Must be highly organized, professional & motivated
Must have working knowledge of nursing services, nursing administration, rehabilitation, general nursing, geriatric nursing
Must have good managerial, organizational, plannings, computer, and interpersonal ability to initiate, complete and update care plans
Job Type: Full-time
Work Location: In person
Pay: $95,000 - $100,000
