About the Role:
We are seeking a compassionate and highly organized Registered Nurse Case Manager to join our hospice care team. This role is ideal for a nurse who values a holistic approach to end-of-life care, thrives in a team-oriented environment, and is dedicated to providing comfort, dignity, and support to patients and their families. Bilingual (English/Spanish) required.
Key Responsibilities:
- Manage a caseload of hospice patients, coordinating care plans and services.
- Conduct comprehensive assessments and develop individualized care plans.
- Provide skilled nursing care, symptom management, and emotional support.
- Collaborate with interdisciplinary team members including physicians, social workers, chaplains, and aides.
- Educate patients and families on disease progression, medications, and care techniques.
- Maintain accurate documentation and ensure compliance with hospice regulations.
- Participate in on-call rotation as needed.
Qualifications:
- Current RN license in Arizona.
- Minimum of 1 year of nursing experience; hospice or palliative care experience preferred.
- Strong organizational and time management skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and as part of a collaborative team.
- Commitment to compassionate, patient-centered care.
- Spanish-speaking required

Pay: $80,000.00 - $95,000.00 per year
