CARE COORDINATION SPECIALIST
We are looking for an out-going individual to join our team as a Care Coordination Specialist. This position requires an individual who is a communicator and people person with the ability to meet deadlines and work independently, while meeting team goals.
The Care Coordination Specialist is responsible for coordinating, monitoring, and supporting services for individuals enrolled in the Arkansas Community and Employment Supports (CES) Waiver. This position ensures that each participant receives person-centered, high-quality services that promote independence, health, safety, and community inclusion while maintaining compliance with Arkansas Medicaid, DDS regulations, and organizational policies.
The Care Coordination Specialist serves as the primary point of contact for participants, families, providers, and community partners to ensure services are delivered according to each participant's Person-Centered Service Plan (PCSP).
Applicant must have excellent written and verbal skills and the ability to manage time well. Knowledge of Word and Excel, reliable transportation and the ability to be on-call are required. A B.A/B.S. in a human services related field is preferred as well.
