American Traveler is seeking an experienced RN for a Home Health position in the Greensboro/High Point, NC area, requiring prior case management experience and HCHB proficiency.

Community-based home health setting — not an inpatient position

Covers the Greensboro and High Point service areas

Role involves case managing an active patient caseload

Monday–Friday, day shift schedule (8:00 AM – 5:00 PM)

13-week contract

Active NC RN license or compact RN license currently in hand

Minimum 1 year of case management experience required

Homecare Homebase (HCHB) EMR experience required

Prior home health travel experience required

Must have a personal vehicle for daily travel throughout the service area

Two professional references required for consideration

PDF copies of driver's license and Social Security card required for consideration

Responsible for managing and coordinating care for an assigned home health patient caseload

Will travel independently between patient homes throughout the Greensboro and High Point areas throughout the workday
