We are looking for a dynamic and compassionate LVN to serve as our Health & Wellness Lead at The Emberly at Heritage Glen. This is a hands-on floor nursing role. You will work directly alongside our Director of Health & Wellness as their right hand, providing expert nursing care and clinical leadership across our Assisted Living and Memory Care neighborhoods every shift.
You own the floor on your shift. That means setting the culture & tone, supporting the care team, and ensuring every resident receives exceptional, personalized care.

Lead A: 4 days per week, 10hour shifts

Lead B: 3 days per week, 10hour shifts

Assist with and oversee medication pass, injections, and treatments per physician orders and Texas nursing practice standards

Conduct resident assessments, vital signs monitoring, and physical evaluations to support individualized care plans

Assist residents with setting up physician visits and serve as the clinical liaison between the care team and medical providers
-
Lead health and wellness programs tailored to residents with memory care, dementia, and chronic disease needs

Complete all clinical documentation including assessments, care notes, MARs, and service plan updates in CMR
-
Manage incident reporting from initial documentation through followup and escalation to the Director of Health & Wellness

Provide health education and coaching to residents and families on medication adherence, nutrition, and disease management
-
Supervise and support care partners on your shift, directing daily care delivery and infection control practices
-
Coordinate with the interdisciplinary team on care transitions, discharge planning, and postacute followup as needed

Support emergency response on your shift per HHSC guidelines

Current, active LVN license in the State of Texas

BLS/CPR certification required prior to first day

Minimum 1 year of nursing experience in assisted living, memory care, longterm care, or a related clinical setting

Handson experience with residents living with Alzheimer's disease, dementia, or memory care needs
-
Solid experience with medication administration and clinical documentation in EHR/EMR systems
-
Strong understanding of pharmacology, vital signs assessment, and care plan development
-
Knowledge of HIPAA compliance standards and infection control practices
-
Working knowledge of Texas ALF regulations and HHSC reporting requirements
-
Strong communication skills with residents, families, physicians, and care team members

Familiarity with ALIS electronic health records
-
CDP certification or willingness to pursue it

Experience in home health, community care, or outpatient settings

Bilingual English/Spanish a plus
