PRIMARY RESPONSIBILITY:
1. Meet and support consumers and families, as applicable, into the program.
2. Establish rapport and provide supportive services to consumers to promote consumer goals.
3. Provide consumer companionship and support while in community activities such as doctor appointments, benefits office, etc.
4. Provide outreach and support team engagement efforts.
5. Assist consumers in transitioning back into the community after hospital discharge, as applicable.
6. Participate in program staff meetings, weekly supervision, treatment planning reviews, and other meetings as needed.
7. Assist consumers in activities of daily living such as grooming and personal hygiene, securing of appropriate clothing, transportation, grocery shopping, and food preparation as needed.
8. Must comply with requirements for EHR documentation based on policies and procedures including updates received via email, trainings, staff meetings, and other written communications.
9. Engage consumers through relationship development, person centered planning, and a focus on their futures.
10. Support consumer’s competencies to assist them in achieving greater self-sufficiency, self-determination, and confidence.
11. Function as a role model of recovery and resiliency for consumers and their families, as applicable.
12. Provide direct service to consumers to include provision of life skills development and 1:1 intervention, as needed, that enhance the consumer’s abilities to participate in activities at their optimal level of functioning.
13. Listen actively to consumer concerns while providing helpful and positive feedback.
14. Adhere to the treatment/recovery plan in keeping with the expressed goals of the consumer and the needs identified by the assessment process.
15. Collaborate with other professional team members by utilizing critical thinking skills, and knowledge to address consumer needs.
16. Assist client and families to develop the Wellness Recovery Action Plan, as appropriate.
17. Perform related work, as necessary or assigned by supervisor.
MINIMUM REQUIREMENTS:
1. Active or past recipient or family member/caregiver of Mental Health Services, as required.
2. Experience in working with mentally ill population.
3. High school diploma or equivalent, may be exempt.
4. Proficiency in the use of computers, email and word processing software.
5. Certified Recovery Peer Specialist (preferred) or Peer Specialist seeking certification, as required.
6. Ability to read, write and speak English, secondary language preferred (Spanish or Creole).
7. A valid Florida driver’s license.
