FT Nights and EVERY weekend (5x8s, 11:00p-7:30a), potential for holiday.
LPN Full Time + every weekend (40 hours)
Required: NY LPN Licensure, BLS (AHA or RQI specifically)
1 year experience as LPN,
Provide direct and indirect nursing care to residents under the supervision of a Registered Nurse
Deliver age-appropriate care primarily to adult and geriatric populations
Support a safe, collaborative, and patient-centered care environment
Collect and report resident physical and psychosocial data to RN and care team
Assist in developing and updating individualized care plans
Monitor resident progress and recommend care plan revisions as needed
Collaborate with interdisciplinary team, residents, and families
Maintain clinical competency through continuing education and training
Support staff orientation and mentoring when applicable
Ensure compliance with safety, infection control, and facility policies
Active New York LPN License (Required)
BLS Certification (AHA or RQI only – Required)
Minimum 1 year LPN experience in a Long-Term Care (LTC) setting
Experience with adult and geriatric patient populations
Ability to work in environments with potential exposure to blood/body fluids
