Pathway to Hope Counseling Services is a Community Behavioral Health Agency, dedicated to providing high-quality mental health services. We are going into our 20th year providing quality services to clients in the community and are fully accredited by CARF. Our agency is currently developing a school-based program to service students in Winder, Ga and surrounding counties.
Summary
The School-Based Program Coordinator serves as the primary operational liaison between Pathway to Hope Counseling Services and participating schools. This position ensures efficient referral management, communication, coordination of services, and timely response to student needs.
Responsibilities:
- Serve as primary day-to-day contact for school personnel.
-Coordinate referrals and service assignments.
-Monitor referral timelines and service initiation.
-Facilitate communication between clinicians and schools.
-Assist with scheduling student services and group interventions.
-Coordinate school-based meetings and program activities.
-Track service utilization and referral trends.
-Support implementation of district-specific procedures.
-Address operational concerns and facilitate resolution.
-Assist with crisis coordination when needed.
Requirements:
-Bachelor's degree required; Master's degree preferred.
-Experience working in education, behavioral health, or human services.
-Strong organizational and communication skills.
-Experience coordinating services across multiple stakeholders preferred.
