Title: Nurse Case Manager
Location: Field-Based – Camden County, NJ, and surrounding areas, including Burlington, Atlantic, and Gloucester Counties
Hours: Monday–Friday, 8:00 AM–5:00 PM
Pay: $44.12 per hour
Responsibilities:
- Conduct comprehensive assessments to identify members’ health needs, eligibility, benefits, and available resources.
- Develop appropriate approaches to case resolution using internal and external programs and services.
- Apply clinical criteria, guidelines, standardized case-management plans, policies, and regulatory standards.
- Coordinate services and advocate for members to promote optimal, cost-effective health outcomes.
- Follow case-management and quality-management processes in compliance with company, regulatory, and accreditation requirements.
- Travel to hospital facilities to meet with members and hospital staff.
- Collaborate with assigned care managers and communicate visit findings promptly and accurately.
Required Qualifications:
- Associate degree or higher.
- Active, unrestricted New Jersey RN license.
- New Jersey residency required; Camden County residency preferred.
- Candidates residing in Burlington, Atlantic, or Gloucester County must be willing to manage a Camden County caseload.
- At least three years of clinical practice experience in a hospital, home health, ambulatory care, or similar setting.
- Healthcare and/or managed-care industry experience.
- Strong computer skills, including navigating multiple systems and keyboarding.
- Effective written and verbal communication skills.
- Ability to multitask, prioritize, and adapt in a fast-paced environment.
- Case-management experience preferred.
- Certified Case Manager (CCM) credential preferred.
