Job Summary
Join our dynamic healthcare team as a Home Infusion Registered Nurse, covering the Eastern Oregon communities (Hermiston, Pendleton in particular), where you will play a vital role in delivering expert nursing care directly to patients in their homes. This energetic position offers the opportunity to provide personalized infusion therapy, manage complex medication administration, and support patients through their treatment journeys. As a key member of our home care services, you will utilize your clinical expertise to ensure safe, effective, and compassionate care while promoting patient independence and well-being.
Responsibilities
- Administer intravenous (IV) infusions, medications, and therapies following established protocols and physician orders, ensuring precise medication delivery and patient safety.
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and evaluation of treatment responses to tailor care plans effectively.
- Manage aseptic techniques during procedures such as catheter care, venipuncture, and sterile dressing changes to prevent infections.
- Educate patients and caregivers on medication management, infusion procedures, device operation, and safety protocols to empower self-care at home.
- Document all patient interactions accurately within electronic health records (EHR/EHR systems), maintaining compliance with HIPAA regulations and medical documentation standards.
- Collaborate with interdisciplinary teams—including physicians, pharmacists, and case managers—to coordinate care plans and ensure seamless service delivery.
- Monitor for adverse reactions or complications related to infusion therapy or medications like botulinum toxin treatments used in ophthalmology or neurology settings; respond promptly to emergencies.
Skills
- Proven experience working with complex medical procedures such as IV infusion, catheterization, tube feeding, and ventilator management.
- Strong venous access skills.
- Strong knowledge of medical terminology, physiology, and aseptic techniques essential for safe patient care.
- Familiarity with electronic charting stems for accurate documentation and communication.
- Critical care experience in settings like ICU or trauma centers is highly desirable for managing acute situations effectively.
- Ability to perform patient assessments, triage calls, and provide health coaching tailored to individual needs across diverse populations including geriatrics, pediatrics, and individuals with disabilities.
- Knowledge of infection control practices.
- Excellent communication skills to educate patients/caregivers and collaborate with healthcare teams efficiently.
- Comfortable working independently in patients' homes.
Qualifications
- Active and unrestricted Registered Nurse (RN) license in Oregon.
- Minimum of 1 year of infusion nursing, IV therapy, or other relevant clinical experience preferred.
- Demonstrated ability to independently start peripheral IVs.
- Strong clinical judgment and ability to work autonomously in the home setting.
- Excellent communication, organization, and documentation skills.
- Current BLS certification required.
- Valid driver's license, reliable transportation, and willingness to travel within the service area.
- Experience with biologics, IVIG, hydration, anti-infectives, or specialty infusion therapies preferred.
