We are looking for a courteous and caring person to join our team and help ensure that our agency can provide patient centered care with an emphasis on maintaining key MCO quality metrics.
Responsibilities
- Coordinate and oversee care that is safe, timely, effective, efficient, equitable, and client-centered
- Complete Intake Paperwork.
- Handle case assignments, draft care plans, review case progress and determine case closure
- Help clients achieve wellness and autonomy
- Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options
- Record cases information, complete accurately all necessary forms and reports
- Adhere to professional standards as outlined by protocols, rules and regulations
- Visit Clients and Prospects to establish relationship with the company.
Skills
- Proven working experience in home care case management
- Excellent knowledge of case management principles, healthcare management and reimbursement
- Previous experience with psychological aspects of care
- Effective communication skills
- Excellent organizational and time management skills
- Familiarity with professional and technical emerging knowledge
- Problem solving skills and ability to multi-task
- Compassionate with teamwork skills
Requirements:
- Candidates must have a car (milage reimbursement)
- Driver's License
