Position Overview
The Clinical Pharmacist provides oversight and safety for all daily operations of the assigned patient care team, including management of assigned pharmacy staff, clinical support to patients, and ensuring that all prescriptions are dispensed in accordance with state, federal, and board of pharmacy laws.
Key Responsibilities
Clinical Patient Care & Counseling: Conducts comprehensive pharmacotherapy reviews (screening for drug interactions and side effects), counsels patients on new therapies, provides subcutaneous administration training, and implements clinical management models.
Care Coordination & Liaison: Serves as the primary intake for phone referrals and acts as a liaison between patients, caregivers, hospice/home health nurses, and physicians to coordinate customized care needs. Dispensing & Operational Compliance: Accurately processes and dispenses prescription orders in strict accordance with federal, state, Medicare/Medicaid, and Board of Pharmacy laws, including controlled substance regulations. Quality Assurance & Safety: Conducts double-check verifications on order entries and final product dispensing, monitors hazardous waste management, and reports adverse events and quality complaints.
Team Supervision & Support: Oversees pharmacy technicians, supervises aseptic techniques, assists with claims adjudication, and provides on-call coverage outside regular operational hours.
Qualifications & Requirements
Education: Graduate of an accredited school of pharmacy.
Experience: 3-5 years or more of post-graduate experience in pharmacy operations, clinical oversight, and direct patient care.
Licensure: Current, unrestricted pharmacy license in good standing. Licensed to practice pharmacy in Texas with the ability to apply for additional state licensures as needed
