Requirements to Register (Florida AHCA Compliance Checklist):
To be listed on our active referral roster, Florida Statute 400.506 mandates that you provide a complete compliance folder including:
- Active, unencumbered Florida RN or LPN License.
- Active AHCA Level 2 Background Screening Clearance via the Care Provider Clearinghouse.
- Physical exam/Health Statement signed by a licensed healthcare professional within the last year stating you are free from communicable diseases.
- Comprehensive professional educational and employment history.
- Valid photo ID (Driver's License).
- Your own Professional Liability Insurance policy.
