About the Role
A growing direct-pay diagnostic imaging practice serving the Charleston, South Carolina area is seeking a South Carolina-licensed physician to provide remote clinical review of non-acute outpatient ultrasound requests.
Patients complete a structured medical intake before requests are submitted for physician review. All requests undergo internal screening and preapproval. Patients with potentially urgent or emergent symptoms are redirected for appropriate urgent or emergency evaluation by the internal team before the request reaches the reviewing physician.
The physician reviews the submitted clinical information, determines whether the requested ultrasound is clinically appropriate, and electronically approves, modifies, or declines the imaging order using independent medical judgment.

Limited Scope of the Position
This is primarily an asynchronous chart-review role.
There are:
- No in-person visits
- No anticipated routine telehealth visits
- No medication prescribing
- No longitudinal patient management
- No responsibility for communicating imaging results to patients
- No responsibility for arranging referrals, additional testing, or treatment
- No responsibility for redirecting urgent or emergent requests
- No ongoing follow-up responsibilities
The practice’s internal clinical and administrative team will manage patient communication, scheduling, results follow-up, referrals, care coordination, and next steps.
A telehealth platform will be available if the reviewing physician believes additional clinical information is required before making a determination. The reviewing physician may request additional information or, when clinically necessary, conduct a brief telehealth encounter to obtain the information needed to complete the medical-necessity review.
Responsibilities
- Review patient-submitted clinical information for requested outpatient ultrasound examinations
- Determine whether the requested study is clinically appropriate
- Approve, modify, or decline requests using independent medical judgment
- Electronically sign approved imaging orders through the provided EMR
- Complete assigned reviews within two hours during weekday business hours
- Request additional clinical information when needed
- Conduct a brief telehealth encounter only when clinically necessary to complete the review
- Document a brief rationale when modifying or declining a request
Expected Volume
- Initial volume is anticipated to be approximately 30 chart reviews per month
- Review volume may increase as the practice grows
- Compensation may be renegotiated by mutual agreement if volume consistently or materially exceeds the anticipated workload
Requirements
- Active, unrestricted South Carolina MD or DO license. Additional licensing in Florida is a plus and offers additional contract opportunities.
- Must carry own current professional-liability or malpractice coverage.
- Board-certified or board-eligible preferred
- Reliable availability during weekday and weekend business hours
- Ability to complete assigned reviews within two hours during business hours
- Comfortable making timely, independent clinical decisions
- Prior telehealth, outpatient, utilization-review, primary-care, emergency-medicine, or urgent-care experience is helpful but not required
Compensation
- $400 fixed monthly retainer
- Compensation covers professional chart-review services and agreed-upon weekday availability
- Compensation is not based on whether a request is approved, modified, declined, or ultimately results in imaging
- Compensation and workload may be renegotiated if monthly review volume consistently exceeds approximately 30 cases
