CAREER TITLE: COMMUNITY HEALTH WORKER (RADT CERTIFIED)
The Community Health Worker provides hands-on care to individuals with varying degrees of mental illnesses and Substance Addiction. Their presence and duties are essential, as they will be positioned at the frontline in all potential client engagement and assistance related situations. We're looking for individuals dedicated to treating our patients with kindness and respect, who can work well in a pulse-pounding environment and make split-second decisions.
TASKS:
- Will complete progress notes documenting pre- and post- service provision in electronic medical record (EMR) in a timely manner and in accordance with standards designated by Program Director and note as well as report any other symptoms from the patient that need immediate professional attention.
- Assist with carrying out educational, recreational, and therapeutic activities for clients including office planned events for clients
- Document patient’s behavior and mental/behavioral/medical/ substance health history.
- Provide a safe space for each patient, while maintaining a supportive and caring environment.
- Interpersonal and observational skills
- Documents and assists with the management of patient’s medications and vitals.
- Serve as the frontline in administering supportive mental and behavioral healthcare and helpful resources.
- Adhere to HIPPA
Essential Job Requirements
- Strong listening, oral, and written communication skills.
- Intermediate computer skills in Microsoft Word, Excel, Outlook and PowerPoint, Office 365, Teams, Zoom and other communication/meeting platforms.
- Basic knowledge and navigation for obtaining California benefits
- Ability to stand and/or walk for long periods of time
Required:
- High school diploma or GED
- Strong communication skills
- CPR /First Aid, Food Handlers Certification (Needed before start date)
- Current TB, and Physical (Needed before start date)
- Mental Health /Substance Abuse Experience
- Current RADT

Pay: $21.29 - $21.96 per hour
Expected hours: 10 – 35 per week
