The Occupational Therapist is responsible for evaluating residents, developing individualized treatment plans, and providing therapy services to help residents improve their ability to perform daily living activities. The OT works closely with physicians, nursing staff, therapy team members, and families to ensure the best possible outcomes for our residents.
- Current Occupational Therapist license in the state of employment
- Graduate of an accredited Occupational Therapy program
- Experience in skilled nursing, rehabilitation, or long-term care preferred
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Evaluate residents and develop individualized occupational therapy treatment plans
- Provide skilled therapy services to improve mobility, strength, coordination, and daily living skills
- Train residents in activities of daily living (ADLs) such as dressing, grooming, and feeding
- Recommend adaptive equipment and environmental modifications to promote independence and safety
- Collaborate with physicians, nursing staff, and the interdisciplinary care team
- Maintain accurate and timely documentation in accordance with facility and regulatory requirements
- Educate residents and families on therapy goals, progress, and home programs
- Participate in care plan meetings and discharge planning
