Compensation: $80 to $120 per Visit
Schedule: Every Weekend or Every Other Weekend Available
Location: Twin Cities Metro Area (21 counties broken up into territories)

What You'll Do
As a Weekend Physical Therapist, you'll evaluate and treat patients recovering from surgery, illness, injury, or chronic conditions throughout the Twin Cities metro.
Responsibilities include:
- Perform comprehensive physical therapy evaluations in the patient's home, assessing mobility, strength, balance, endurance, safety, and functional limitations
- Develop individualized treatment plans that reflect each patient's clinical needs, personal goals, home environment, and physician orders
- Provide skilled interventions designed to improve strength, gait, transfers, balance, range of motion, endurance, and overall functional independence
- Complete reassessments and update plans of care based on patient progress, changes in condition, and continued need for skilled therapy services
- Identify fall risks and provide practical recommendations to improve safety throughout the home
- Recommend and educate patients on the appropriate use of walkers, canes, wheelchairs, and other durable medical equipment
- Educate patients and caregivers on home exercise programs, transfer techniques, mobility strategies, and ways to safely support continued progress
- Collaborate with nurses, occupational therapists, speech therapists, social workers, physicians, and other care team members to support coordinated patient outcomes
- Communicate changes in patient condition promptly and appropriately to the interdisciplinary care team
- Complete accurate and timely electronic documentation that reflects the patient's condition, treatment provided, progress, and ongoing clinical needs

Qualifications
Required
- Current Minnesota Physical Therapist license in good standing
- Current CPR certification
- Valid driver's license
- Reliable transportation and the ability to travel between patient homes
- Strong clinical assessment, communication, time-management, and documentation skills
- Ability to work independently and make sound clinical decisions in a home-based setting
Preferred
- Previous home health, senior care, rehabilitation, skilled nursing, or geriatric therapy experience
- Familiarity with Medicare home health requirements and electronic medical record systems
- Experience working with patients recovering from orthopedic procedures, hospitalization, falls, neurological conditions, or chronic illness
