The Role
Vista is seeking a Case Coordinator (Case Manager) to provide a combination of direct support and case coordination services for identified Engage members. The Case Coordinator maintains an assigned caseload and serves as the primary point of contact for members, families, guardians, support teams, and community partners.
Approximately 30 hours per week are dedicated to direct service delivery and participant coordination. Remaining scheduled hours are dedicated to administrative duties, supervision, documentation, meetings, family communication, and indirect service coordination.
What You’ll Do
- Support students and members in developing independence across life skills, social, cognitive, vocational, and community-based areas through individualized instruction and skill-building strategies.
- Develop, implement, and monitor written program plans, including IPPs and IPs, that identify goals, service needs, progress, and individualized supports.
- Coordinate and monitor assigned caseload needs by communicating with team members, families, departments, and other involved parties to ensure consistent support and service delivery.
- Provide direct instruction, coaching, and counseling related to social skills, cognitive development, medication awareness, money management, time management, work readiness, goal setting, and performance review.
- Provide direct supervision to Life Skills Instructor(s).
- Create and implement procedures to assist other Life Skills Instructors to use assistive technology to assist students with gaining independence.
- Evaluate a student/member’s progress and provide appropriate training interventions.
What We’re Looking For
- Bachelor’s degree plus one year of related experience.
- Valid driver’s license and maintain acceptable driving record as defined by Vista.
- CPR, First Aid and PMT certified or the ability to obtain certifications within 90 days of hire.
- Transport students and members in either personal or Vista car/van to community sites in a variety of settings and weather conditions.
- Knowledge and skills in MS Office, including Outlook and the internet.

Salary: $56,100 annually
Location: Madison, CT | Status: Full-time, Exempt, Monday-Friday 9am-5pm
