Care Coordinators provide support services to children and adult clients through ongoing assessment, monitoring, coordination of services, referrals, and treatment planning. PCS participates in the state's care coordination program, Tennessee HealthLink, which aids in bridging the gap between mental and physical health.
Qualifications: Must have a Bachelor's degree with a minimum of 15 credit hours in a social services discipline such as social work, psychology, counseling, human services, or a related field.
