Position Summary
The Dream Works is seeking compassionate, dependable individuals to join our team as Transitional Home & Community Support (THCS) Specialists.
In this role, you will work one-on-one with individuals who have experienced a traumatic brain injury (TBI), helping them develop or regain the skills needed to live as independently as possible. Your support will focus on improving physical, cognitive, behavioral, and daily living skills while promoting greater independence at home and within the community.
Responsibilities
- Teach and reinforce independent living and household management skills.
- Develop strategies that help individuals compensate for cognitive and physical challenges.
- Provide hands-on instruction and support to individuals and their families.
- Assist individuals in achieving goals identified in their service plans.
- Maintain accurate, timely documentation of services provided.
- Provide services in clients' homes and throughout the community.
- Communicate professionally with supervisors, team members, and other service providers while maintaining confidentiality.
Preferred Qualifications
- High school diploma or GED required.
- Experience working with individuals with disabilities, brain injuries, behavioral health, or human services is preferred but not required.
- Strong organizational and time management skills.
- Excellent communication and interpersonal skills.
- Critical thinking, problem-solving, and decision-making abilities.
- Basic computer and technology skills, including email, internet, smartphones, and electronic documentation.
- Commitment to maintaining client confidentiality and professional boundaries.
Requirements
Applicants must:
- Possess a valid driver's license.
- Have reliable transportation (personal vehicle) and current auto insurance.
- Be willing to travel throughout the Kansas City metro area.
- Hold current Adult First Aid and CPR certification or be willing to complete employer-provided training.
- Attend all required trainings.
- Successfully pass reference checks, employment verification, drug screening, and background checks.
- Have regular access to a computer and the internet for documentation and communication.
