Responsibilities
- Provide attentive assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health through observation of vital signs and overall condition, promptly reporting any concerns to healthcare professionals
- Assist with medication reminders following prescribed schedules and medical documentation protocols while ensuring HIPAA compliance
- Support clients with specialized needs such as dementia, Alzheimer's, memory care, or hospice care by implementing personalized care plans
- Operate Electronic Health Record (EHR) and Electronic Medical Record (EMR) systems accurately to document patient information and medical records
- Use medical terminology confidently when communicating with healthcare providers or documenting patient status
- Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and maintaining a safe living environment
- Safely utilize equipment like Hoyer lifts and perform catheter care when necessary to ensure patient comfort and safety
- Assist with medical procedures including vital signs measurement, wound care, or hospice-related activities as trained
- Maintain confidentiality and adhere strictly to HIPAA regulations while managing sensitive patient information
Skills
- Proven experience in home care or long-term care environments with a focus on senior or disability care
- Strong knowledge of patient observation techniques and ability to recognize changes in health status
- Familiarity with assisted living settings, nursing homes, or similar facilities is highly desirable
- Competence in working with individuals with developmental disabilities or dementia-related conditions such as Alzheimer's disease
- Ability to operate EMR/EHR systems efficiently for accurate medical documentation and record keeping
- Knowledge of medical terminology, medication administration protocols, and basic nursing skills including CPR certification
- Skilled in assisting with ADLs such as feeding, grooming, mobility support using Hoyer lifts or other assistive devices
- Experience working within HIPAA guidelines to ensure patient privacy and confidentiality at all times
- Capable of performing household chores like cleaning, laundry, cooking nutritious meals tailored to dietary needs
- Excellent communication skills for collaborating effectively with clients’ families and healthcare teams
