SUMMARY
Participate in the assessment and treatment of the clinical needs of clients. Assist in the development of appropriate treatment plans and assume overall psychiatric case management responsibilities for assigned clients.
DUTIES AND RESPONSIBILITIES
- Conduct initial screening and comprehensive assessments of clients for mental health services.
- Provide individual and group counseling services.
- Develop initial treatment plans and complete timely updates based on client progress.
- Assess client needs and provide appropriate referrals to supportive services.
- Deliver crisis intervention services as needed.
- Develop discharge plans and coordinate appropriate referrals upon completion of services.
- Assume overall psychiatric case management responsibilities for assigned clients, including coordination of care and communication with treatment providers.
- Collaborate effectively with staff, referral sources, and social support networks to ensure comprehensive care for clients.
- Maintain effective and open communication with members of the treatment team.
- Complete and maintain accurate client records, charts, and reports in a timely manner.
- Stay current with trends and developments in the mental health field by attending meetings, trainings, workshops, and seminars.
- Maintain knowledge of all applicable federal, state, and NJCRI regulations, laws, and policies, including updates and modifications.
- Perform other duties as assigned.
QUALIFICATIONS
EDUCATION and/or EXPERIENCE
Bachelor’s Degree in mental health or related field. Excellent leadership, interpersonal skills and creative problem-solving skills.
Pay: $68,000.00 - $70,000.00 per year
